On or about May 6, 2020, located at 6953 Hollyhock Drive (Lot #10) in New Albany, Ohio, an employee was installing roofing shingles without a fall arrest system, thereby exposing the employee to a fall between approximately 11 to 20 feet. b. On or about May 6, 2020, located at 6953 Hollyhock Drive (Lot #10) in New Albany, Ohio, an employee was installing roofing shingles while the fall protection rope was tied around a single roofing truss, thereby exposing the employee to a fall of approximately 11 feet.
