29 CFR 1926.102(a)(2): The employer did not ensure that each affected employee used eye protection that provided side protection when there is a hazard from flying objects:  Residential Construction Site: On or about May 8, 2020, and at times prior thereto, employees were exposed to a struck-by flying projectile hazard, while using a Makita Circular Saw (model 5007F) and Makita Pneumatic Gun without safety glasses.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary to enable each employee to recognize hazards related to ladders and stairways, and the procedures to be followed to minimize these hazards:  East side of the new house construction site. On or about May 8, 2020 and at times prior thereto, employees were exposed to a fall hazard while not using a ladder to access the roof.
