ESTABLISHMENT: On or about April 7, 2020, the employer had not established and implemented a written worksite-specific respiratory protection program and procedures consisting of employee training, fit testing, proper selection, and medical evaluations where employees were required to wear KN-95 filtering face piece respirators during work tasks such as (but not limited to) providing patient care for residents who were COVID-19 positive or were suspected of having COVID-19
