29 CFR 1910.22(a)(2): The employer did not ensure the floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. When wet processes are used, drainage was not maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats were not provided:  a) Wayfield Foods grocery store, Meat Department: On or about 05/12/2020, the employer exposed employees to slip hazards in the meat department due to wet floors, in that floors were not kept in a dry condition as floor drains were covered with mats and the sink in need of repairs was leaking onto the walking/working surface creating a hazard.

29 CFR 1910.22(d)(2): The employer did not ensure hazardous conditions on walking-working surfaces were corrected or repaired before an employee used the walking-working surface again.  When the correction or repair cannot be made immediately, the hazard was not guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired:  a) Wayfield Foods grocery store, Meat Department: On or about 05/12/2020, the employer exposed employees to trip hazards in the meat department from uncovered floor drains and damaged floors at the freezer, in that floor drain covers and repairs to the freezer entrance surface were not repaired as employees used this walking/working surface daily.

29 CFR 1910.213(i)(1): Non-working portion(s) of the blade of bandsaw(s) were not enclosed or guarded:  a) Wayfield Foods grocery store, Meat Department: On or about 05/12/2020, the employer exposed employees to severe laceration and amputation hazards while using the Hobart (Model 6614) Meat Saw, in that the blade guard was not operable and unable to be used on the non-working portion of the blade when used to cut beef.   b) Wayfield Food grocery Store, Meat Department: On or about 05/12/2020, the employer exposed employees to severe laceration and amputation hazards using the Biro (Model 3334) Meat Saw, in that the blade guide at the base was damaged which caused the blade to jump while cutting as the nylon filler piece was damaged and not replaced.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws  a) Wayfield Foods grocery store, Meat Department Meat Saw #1: On or about 05/12/2020, the power cable for the Hobart (Model 6614) Meat Saw had a internal insulated circuits exposed, in that the strain relief was damaged and not adequately repaired when first damaged as the electrical tape used prior to inspection failed.
