29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about May 13, 2020, at a job site located at 797 E. Market St, Akron, Ohio 44305; during the installation of a commercial HVAC unit, the employer did not ensure all employees using an aerial lift were protected from fall hazards approximately 30 feet by a fall protection device.
