29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). a. On or about May 12, 2020 at the Logan City Pool, 470 N Market St. in Logan, Ohio, two employees were working inside an aerial basket lift without proper fall protection exposing the employees to an approximately 6 foot fall hazard.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a. On or about May 15, 2020 at 470 N Market St, in Logan, Ohio employees were installing shingles on a 6/12 pitch roof on the pool house with no fall protection exposing employees to a ten foot fall hazard.
