29 CFR 1910.147(f)(3)(i): When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device.  a) On or about 05/13/2020, on the outside of the south side of the main building, when maintenance employees were servicing the Torit Dust Collector, a group lock out procedure was not followed to ensure every employee performing this maintenance task applied their own lock to the group lockout device. The temporary employee did not have their lock on the electrical disconnect switch.   ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.147(c)(6)(i): The employer's periodic inspection of the energy control procedure did not ensure that the procedure and the requirement of this standard were being followed.  a) On or about 5/13/2020, on the outside of the south side of the main building, the periodic inspection of energy control procedures did not ensure that the Torit Dust Collector LOTO specific procedures were correct. The energy control procedures of the Torit Dust Collector indicated the wrong electrical disconnect switch potentially exposing employees to serious injuries when servicing this Dust Collector.  ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:  a) On or about 05/13/2020 on the outside of the south side of the main building, the electrical disconnect switch mounted on the Torit Dust Collector was not marked or labeled to indicate the purpose, thereby exposing employees to electrical hazards.
