29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  110 King Arthur Drive, Lagrange, GA 30240, Trench in middle of the public road: On or about 05/13/2020, Employees working in and around an excavation that measured 3-feet, 6-inches wide by 6-feet, 6-inches deep by 20-feet long were not provided adequate knowledge and training to recognize the hazards associated with trenching and excavation and the means to control or eliminate those hazards.

29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:  (a) 110 King Arthur Drive, Lagrange, GA 30240, Trench and Trench Shield: On or about 05/13/2020, The employer failed to inspect the excavation and the cave-in protection systems continuously during the shift, in that trench shield spreaders and keeper pins were not correctly inserted and or missing on the collars of the trench shield wall panels exposing employees to struck-by hazards in and around the excavation.
