29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: (a) 110 King Arthur Drive, Lagrange, GA 30240, Trench and Trench Shield: On or about 05/13/2020, The employer failed to inspect the excavation and the cave-in protection systems continuously during the shift, in that trench shield spreaders and keeper pins were not correctly inserted and or missing on the collars of the trench shield wall panels exposing employees to struck-by hazards in and around the excavation.
