Subcontractor employees were not wearing eye protection while using pneumatic nail guns used for re-roofing activities.  This condition exposed subcontractor employees to struck-by hazards from projectiles.

Pontiac Way, Denver, CO 80224: On or before May 19, 2020, the controlling employer did not ensure that subcontractor employees engaged in roofing activities were wearing proper fall protection.  25 subcontractor employees were working next to the edge of the roof, on a single story, commercial storage building without fall protection.  5 storage buildings with a 6/12 pitch; were being re-roofed by the subcontractor.  Subcontractor employees were not utilizing fall protection in a manner that would prevent injury, subcontractor employees were not trained in proper fall protection requirements, fall protection equipment was not being used within manufacturers recommendations and multiple subcontractor employees were tied into single anchor points engineered for one person. This condition exposed subcontractor employees to fall from height hazards of 8'4" to 16' feet to the asphalt below.

OSHA 300 logs were requested for 2018, 2019 and 2020(partial) and OSHA 300A Summary Logs were requested for 2015, 2016, 2017, 2018 and 2019. Records were not provided for 5-7 days after they were requested.
