The Datsun forklift did not have a functional seat belt or horn.

The lower pulley required additional guarding for the exposed section that was approximately 9 inches in length and 3-1/2 inches in width.

29 CFR 1910.303(b)(7)(iv): Electrical equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating: (a) Employees engaged in grain processing work along the north side of bin #2 at 3708 N Free King Highway, Pittsburg, Kansas were exposed to electrical shock hazards created by three, 1 inch diameter conduits being broken, thus exposing energized conductors. (b) Employees engaged in grain processing work along the south side of bin #11 at 3708 N Free King Highway, Pittsburg, Kansas were exposed to electrical shock hazards created by the 1 inch diameter conduits being broken that was entering on the right side of panel box #5, thus exposing energized conductors. (c) Employees engaged in grain processing work along the north side of bin #12 at 3708 N Free King Highway, Pittsburg, Kansas were exposed to electrical shock hazards created by two 1 inch diameter conduits being broken, thus exposing energized conductors.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): Employee(s) engaged in using electrical powered tools located at 3708 N. Free Kings Highway, Pittsburg, Kansas were exposed to electrical shock hazards due to the damaged outer jacket of powered electrical tool cords not being visually inspected prior to use for the following equipment: (a) V.S. reciprocating saw power cord was damaged exposing conductors. (b) Ryobi circular saw power cord was damaged exposing conductors. (c) Hitachi 4-1/2 inch portable disc grinder had electrical tape on a section of the power cord that was not maintaining the characteristics of the original outer jacket covering the conductors. (d) DeWalt 4-1/2 inch angle grinder had electrical tape on a section of the power cord that was not maintaining the characteristics of the original outer jacket covering the conductors.
