29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). a) Jobsite: 112 Railroad Ave, Oxford, PA 19363, Stone lot in New Street, box van - Employees inside a box truck handling a load brought in by a powered industrial truck were exposed to struck-by and caught in-between hazards in that the employer did not ensure each operator was competent and trained as required by the standard to operate powered industrial trucks safely. Noted on or about 5/22/20.

29 CFR 1910.178(n)(6):Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel. a) Jobsite: 112 Railroad Ave, Oxford, PA 19363, Stone lot on New Street: An employee operating a powered industrial truck was not required to keep a clear view of the travel path while moving an oversized load into a box van. Employees inside a box truck handling a load brought in by a powered industrial truck were exposed to struck-by and caught in-between hazards. Noted on or about 5/22/20.
