29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): (a) In the Repack department, for the sauce repacking machine, the specific procedures failed to clearly identify all of the specific steps to be followed by employees to isolate and disable the equipment and control the hazardous energy. The procedure also lacked specific steps to relieve hazardous residual energy contained in the equipment and specific verification steps were not prescribed to determine the effectiveness of the energy control measures. (b) In the Repack department, for the rice repacking machine, the specific procedures failed to clearly identify all of the specific steps to be followed by employees to isolate and disable the equipment and control the hazardous energy. The procedure also lacked specific steps to relieve hazardous residual energy contained in the equipment and specific verification steps were not prescribed to determine the effectiveness of the energy control measures.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) On or about May 18, 2020 and times prior thereto, on the sauce repacking machine, the employer developed a written energy control procedure for the cleaning, maintenance, changing mixer blades, and other servicing activities. The employer did not affix a lockout or tagout device when performing cleaning, unjamming and other maintenance activities. This practice exposed employees to moving machine parts of the mixer blades and cutter blades. (b) On or about May 26, 2020 and times prior thereto, on the rice repacking machine, the employer developed a written energy control procedure for the cleaning, maintenance, changing mixer blades, and other servicing activities. The employer did not affix a lockout or tagout device when performing cleaning, unjamming and other maintenance activities. This practice exposed employees to moving machine parts of the mixer blades and cutter blades.
