29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions: (a) Pipeline Installation Site, 1516 Paw Paw Creek Road: On May 24, 2020 and times prior, employees were exposed to struck by and crushing injuries while traveling up and down hills in a Can-Am Defender XT UTV during pipeline installation. The employer did not ensure appropriate protective equipment such as a DOT approved helmet was utilized while operating the vehicle.
