29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about May 28, 2020, the employer did not ensure that a ladder utilized to access the roof of a residential home located at 2608 Bishop Oval, Barberton, OH 44204 was utilized properly in that it was only extended 1.7 feet above the upper landing surface, was not secured, and did not incorporate a grasping device.
