29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.   a) Wilson Morquecho, at a jobsite located at 5091 Sycamore Trail, Galena, Ohio 43021: On and/or preceding May 26, 2020, the employer failed ensure employees wore head protection while exposed to a head injury from impact from falling or flying objects. Employees were working at ground level while other employees were installing roof sheathing while using a powered nail gun and hand tools. The employees were not utilizing head protection, which exposed them to struck-by hazards.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   a) Wilson Morquecho, at a jobsite located at 5091 Sycamore Trail, Galena, Ohio 43021: On and/or preceding May 26, 2020, the employer did not ensure employees wore safety glasses while utilizing power tools that presented struck-by hazards to the eyes and face. Carpenters were installing roof sheathing while using a powered nail gun and circular saw. The employees were not utilizing eye protection, which exposed them to struck-by hazards to the eyes.

29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.   a) Wilson Morquecho, at a jobsite located at 5091 Sycamore Trail, Galena, Ohio 43021: On and/or preceding May 26, 2020, the employer failed to initiate and maintain a safety and health program such as but not limited to a personal protective equipment program, an electrical safety program, and a fall protection program for employees conducting framing work.  Wilson Morquecho was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1406440, citation number 1, item number 1 and was affirmed as a final order on July 16, 2019, with respect to a workplace located at 2209 Red Oak, Lewis Center, OH 43035.

Employees conducting framing work were exposed to struck-by hazards, fall hazards, and electrical contact hazards.  Wilson Morquecho was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1406440, citation number 1, item number 2 and was affirmed as a final order on July 16, 2019, with respect to a workplace located at 2209 Red Oak, Lewis Center, OH 43035.

The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.  a) Wilson Morquecho, at a jobsite located at 5091 Sycamore Trail, Galena, Ohio 43021: On and/or preceding May 26, 2020, the employer did not ensure employees who used extension cords were protected from electrical contact hazards. Framers were using an extension cord with the ground prong missing from the male end of the cord, which exposed the employees to a 125 volt electrical contact hazard.  Wilson Morquecho was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1406440, citation number 1, item number 3 and was affirmed as a final order on July 16, 2019, with respect to a workplace located at 2209 Red Oak, Lewis Center, OH 43035.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   a) Wilson Morquecho, at a jobsite located at 5091 Sycamore Trail, Galena, Ohio 43021: On and/or preceding May 26, 2020, the employer did not ensure employees were protected from falls while engaged in residential construction activities. Employees were sheathing and framing the porch roof without any means of fall protection, thereby exposed to fall hazards of up to 15 feet.  b) Wilson Morquecho, at a jobsite located at 5091 Sycamore Trail, Galena, Ohio 43021: On and/or preceding May 26, 2020, the employer did not ensure employees were protected from falls while engaged in residential construction activities. Employees were working on the first floor where the stairwell opening to the basement was not guarded, thereby exposed to fall hazards of up to 15 feet  Wilson Morquecho was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1406440, citation number 1, item number 5 and was affirmed as a final order on July 16, 2019, with respect to a workplace located at 2209 Red Oak, Lewis Center, OH 43035.
