Employees were working at ground level while other employees were installing roof sheathing while using a powered nail gun and hand tools. The employees were not utilizing head protection, which exposed them to struck-by hazards.

Carpenters were installing roof sheathing while using a powered nail gun and circular saw. The employees were not utilizing eye protection, which exposed them to struck-by hazards to the eyes.

Employees were working on the first floor where the stairwell opening to the basement was not guarded, thereby exposed to fall hazards of up to 15 feet Wilson Morquecho was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1406440, citation number 1, item number 5 and was affirmed as a final order on July 16, 2019, with respect to a workplace located at 2209 Red Oak, Lewis Center, OH 43035.
