29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a. On or about 05/28/2020, at the job site - employees removing old roofing during a roof replacement job were exposed to falling through holes, in that, skylight were not covered and / or fall protection was not used. An employee fell 13 feet 8 inch to the ground after stepping or standing on a fiberglass skylight.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a. On or about 05/28/2020, at the job site - employees removing old roofing during a roof replacement job were exposed to a 13 feet fall hazard, in that, fall protection was not used.
