29 CFR 1926.453(b)(2)(vi): Boom and basket load limits specified by the manufacturer were exceeded: On or about May 7 2020, employees were working in the baskets of articulated boom lifts where the baskets' rated weight tolerance was exceeded by personnel and a combination of equipment including, but not limited to tools, materials, and welders. The rated weight capacity of the baskets of the articulated boom lifts in use was 500 lbs. and the average weight in and attached to the basket exceeded this by approximately 30-40 lbs.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of aerial lift being used and to understand the procedures to control or minimize those hazards: On or about May 8 2020 employees working from an aerial lift were not trained in the recognition and avoidance of hazards associated with aerial lift usage such as but not limited to overhead hazards, electrical hazards from welding, and tipping hazards.
