29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   a) Exterior platform near the rear stairway of the North Main Structure: On or about May 1, 2020 and at times prior thereto, the employer did not ensure fall protection was used when walking on the platform with unprotected edges which exposed employees to a fall of 15-feet.    b) Exterior platform near the rear stairway of the South Main Structure: On or about May 1, 2020 and at times prior thereto, the employer did not ensure fall protection was used when walking on the platform with unprotected edges which exposed employees to a fall of 15-feet.

29 CFR 1926.501(b)(10): Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  a) Roof surfaces for the two towers of the North Main Structure: On or about May 1, 2020 and at time prior thereto, the employer did not ensure fall protection was used while installing Dura-Last roofing system roof surfaces of the towers which exposed employees to falls up to approximately 38-feet.    b) Roof surfaces for the two towers of the North Main Structure: On or about May 1, 2020 and at time prior thereto, the employer did not ensure fall protection was used while installing Dura-Last roofing system roof surfaces of the towers which exposed employees to falls up to approximately 38-feet.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  a) Rear Stairway of the North Main Structure: On or about May 1, 2020 and at times prior thereto, the employer did not ensure a stairrail system was installed on the open side of each level of the stairway which exposed employees to falls up to 14-feet.  a) Rear Stairway of the South Main Structure: On or about May 1, 2020 and at times prior thereto, the employer did not ensure a stairrail system was installed on the open side of each level of the stairway which exposed employees to falls up to 14-feet.

29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with guardrail systems that meet the criteria contained in Subpart M of 29 CFR 1926:  a) Rear Stairway Upper Landing of the North Main Structure: On or about May 1, 2020 and at times prior thereto, the employer did not ensure guardrails or an equivalent fall protection system was used for the unprotected edges of the landing which exposed employees to falls up to 14-feet.  b) Rear Stairway Upper Landing of the South Main Structure: On or about May 1, 2020 and at times prior thereto, the employer did not ensure guardrails or an equivalent fall protection system was used for the unprotected edges of the landing which exposed employees to falls up to 14-feet.
