29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Facility, 2850 Perry Highway, Slippery Rock, PA: On or about June 1, 2020 no guard was in place in the area where cheese blocks are loaded into the grinder/auger area to protect operators from the rotating parts. (b) Facility, 2850 Perry Highway, Slippery Rock, PA: On or about May 21, 2020 adequate guarding was not in place on the hammer mill to prevent contact with the rotating parts. An employee reached into the chute to remove cheese and made contact with the rotating part sustaining an amputation to the tip of the left middle finger.
