29 CFR 1926.96: The employer did not ensure employees wore proper footwear which meets the requirements and specifications in American National Standard for Men's Safety-Toe Footwear, Z41.1-1967. On or about 5/20/2020, the jobsite located at 757 West Ave. Miami, FL 33139, an employee was exposed to a struck-by hazard when performing debris removal work while wearing sneakers and not using protective footwear.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about 5/20/2020, the jobsite located at 757 West Ave. Miami, FL 33139, employees working on the edge of a roof were exposed to a fall hazard of approximately 120 feet above ground level without a means of fall protection.
