29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a): Facility - Tiger TG-6 Machine - Procedures for the control of potentially hazardous energy were not utilized. An employee was providing maintenance to the machine and suffered serious injuries when he was caught in between a moving metal pallet and the frame of the machine. The guard was open and the employee was working in the danger zone without following the developed procedures for the control of potentially hazardous energy, on or about May 30, 2020.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
