29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A) Exterior on the ground: An employee was exposed to head injuries while working below other employees that were working on a scaffold above him. The employee was not protected by a hard hat or protective helmet, on or about May 28th, 2020.   NOTE: Because abatement of this violation is already documented in the inspection case file, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.                                                                                           A) Exterior, On the scaffold and the ground below: Employees were exposed to eye/face injuries while using an electric saw (ground) and a pneumatic nail gun (scaffold) and were not protected with any type of eye/face protection, on or about May 28th, 2020.   NOTE: Because abatement of this violation is already documented in the inspection case file, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

29 CFR 1926.451(b)(1):     Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports:   A) Exterior, On the scaffold: An employee was exposed to fall hazards ranging from 6 to 24 feet while working on a fabricated framed scaffold that was not fully planked, on or about May 28th, 2020.    NOTE: Because abatement of this violation is already documented in the inspection case  file, the employer need not submit certification or documentation of abatement for this  violation as normally required by 29 CFR 1903.19.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.   A) Exterior, On the scaffold: Employees were exposed to fall hazards from approximately 6 to 24 feet while climbing the scaffold components and were not provided with a safe an acceptable means of access, on or about May 28th, 2020.    NOTE: Because abatement of this violation is already documented in the inspection case  file, the employer need not submit certification or documentation of abatement for this  violation as normally required by 29 CFR 1903.19.
