29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a. Forsyth County Water Treatment Plane (Expansion) - 2225 Antioch Road, Cumming, GA 30040 - Employees were exposed to tip-over hazard. The employer did not instruct employees in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate hazardous conditions associated with identifying and/or verifying that the locks on the castors were securely locked in position when working on a 10 degree sloped porous working floor surface, on or about June 2, 2020.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a. Forsyth County Water Treatment Plane (Expansion) - 2225 Antioch Road, Cumming, GA 30040 - Employees were exposed to tip-over hazard. The employer failed to ensure that the 2-buck high (12ft.) tubular welded fabricated frame scaffold was properly inspected, after changes were made to the scaffold footing such as, changing out the flat mud sill to castor wheels, on or about June 2, 2020.
