29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   9707 Madison Avenue, Webster Groves: Employer is exposing employees to fall hazards.  Employer is exposing employees to fall hazards.  The employees are permitted to work on residential roofs higher than 6 feet from ground level without using fall a protection system.  On 06/03/2020 employees were exposed to a fall distance of greater than 11 feet when they worked on a residential roof without using any fall protection systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows for the ladder side rails to be extended, the ladder's side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:      9707 Madison Avenue, Rock Hill: Employer is exposing employees to fall hazards.  The employees are permitted to access residential roofs higher than 6 feet from ground level using ladders.  Side rails of the ladders are not extended at least 3 feet above the upper landing surface.     On 06/03/2020 employees were exposed to a fall distance greater than 11 feet when they accessed a residential roof      using a ladder.  The side rails of the ladder extended approximately 1 foot above the roof.
