29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.   At the knurler machine #5, the employer did not equip the machine with a two hand tripping device to ensure employees were not able to place their hands inside the downward travel of the tube guard, exposing employees to struck-by and amputation hazards.

On or about May 8, 2020 and at times thereafter, the employer did not report a work place amputation/hospitalization within the required 24 hours after the incident or when the employer was first aware about the incident.

29 CFR 1910.217(f)(2): Supervision to ensure that correct operating procedures were being followed on mechanical power press(es) was inadequate.     At the knurler machine #5, the employer did not supervise employees to ensure operating procedures of machines were followed in a safe manner exposing employees to amputation hazards.
