29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: (a) Truck Maintenance Shop: On or about June 4, 2020 a Sentry ABC portable fire extinguisher was sitting unsecured on an elevated small diameter pipe directly next to a doorway and garage door. Employees were exposed to a projectile hazard if the extinguisher, a compressed gas cylinder, was knocked to the floor damaging the valve head.

29 CFR 1910.157(e)(1): The employer did not take responsibility for the inspection, maintenance and testing of all portable fire extinguishers in the workplace: (a) Truck Maintenance Shop: On or about June 4, 2020 the employer did not assure that all portable fire extinguishers present on site had been subjected to an annual maintenance check when the employer moved into the site in that the Sentry portable dry chemical fire extinguisher had not been through an annual maintenance check since it was put in service in 2016. (b) Truck Maintenance Shop: On or about June 4, 2020 the employer did not assure that all portable fire extinguishers present on site had been subjected to an annual maintenance check when the employer moved into the site in that the Ansul portable dry chemical fire extinguisher serial number CD887046 did not have any evidence of when the last annual maintenance check had been conducted.
