This was most recently documented on June 4, 2020 at the workplace located at 8270 Nebraska Avenue, Building 5265, Fort Leonard Wood, Missouri 65473. The cab of the Grove YB 4415 did not have a load rating chart with clearly legible letters and figures, easily visible to the operator while seated at the controls.

29 CFR 1910.180(h)(3)(i)(b): The employer did not assure that the load was well secured and properly balanced in the sling or lifting device before it was lifted more than a few inches.  The employer is failing to protect employees from unsecured and improperly balanced crane loads. This was most recently documented on June 4, 2020 at the workplace located at 8270 Nebraska Avenue, Building 5265, Fort Leonard Wood, Missouri 65473. The employer did not assure that the broken piece of equipment from the M1120 HEMTT LHS was well secured and properly balanced with the lift chain before employees moved it more than a couple of inches off the ground with a Grove 15 ton telescoping boom crane.

The employer is failing to protect employees from electrocution hazards associated with overhead power lines. This was most recently documented on June 4, 2020 at the workplace located at 8270 Nebraska Avenue, Building 5265, Fort Leonard Wood, Missouri 65473. Logistic Readiness Center employees were conducting crane operations near overhead power lines. The power lines were not deenergized and grounded, nor were other protective measures provided before crane operations began.

The employer is failing to protect employees from electrocution hazards associated with overhead power lines. This was most recently documented on June 4, 2020 at the workplace located at 8270 Nebraska Avenue, Building 5265, Fort Leonard Wood, Missouri 65473. Employees were operating a Grove YB4415 15 ton telescoping boom crane near energized overhead power lines, without maintaining a 10 foot clearance from the power lines and the boom of the crane.

29 CFR 1910.333(c)(3)(iii)(B): Employees standing on the ground may not contact the vehicle or mechanical equipment or any of its attachments, unless: the employee is using protective equipment rated for the voltage; or the equipment is located so that no uninsulated part of its structure (that portion of the structure that provides a conductive path to employees on the ground) can come closer to the line than permitted in paragraph (c)(3)(iii) of this section.  The employer is failing to protect employees from electrocution hazards when not providing proper personal protective equipment. This was most recently documented on June 4, 2020 at the workplace located at 8270 Nebraska Avenue, Building 5265, Fort Leonard Wood, Missouri 65473. An employee who was assisting in crane operations made contact with the load without being equipped with personal protective equipment rated for the voltage of the overhead power lines. The Grove YB4415 15 ton telescoping boom crane was not located to prevent uninsulated parts of the crane structure to get closer than 10 feet from energized overhead power lines.

29 CFR 1960.8(a): 29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm:  The employer is failing to protect employees from electrocution hazards associated with overhead power lines. This was most recently documented on June 4, 2020 at the workplace located at 8270 Nebraska Avenue, Building 5265, Fort Leonard Wood, Missouri 65473. Logistic Readiness Center employees were conducting crane operations near overhead power lines. The employer failed to determine if any part of the equipment, load line or load (including rigging and lifting accessories), if operated up to the equipment's maximum working radius in the work zone, could get closer than 20 feet to a power line.  Among other methods, one feasible and acceptable abatement method is to:   1. Assess the job and evaluate the equipment being utilized, which would remove employees from external hazards.   2. Complete a work site hazard assessment in accordance with the Grove Operator's and Safety Handbook Yardboss 4415 to evaluate equipment being utilized, the area equipment is utilized in, hazards within the equipment operating areas, such as overhead power lines, and ensure appropriate measures are taken to protect employees operating and/or working in proximity to the equipment.
