On June 4, 2020, at a worksite located at 4007 Birchwood Dr, Kansas City, MO. An employee was exposed to serious injuries, in that there was standing water which continued to accumulate in the open excavation approximately 8 feet in depth due to an in-effective Sump Pump at the work site.

An employee working in an 8 ft excavation was exposed to serious struck by and or engulfment hazard, in that there was no protective systems in use for work being done in the water saturated excavation.
