29 CFR 1910.303(b)(1):   Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:  On June 4, 2020, at a worksite located at 4142 S Noland, Independence, MO. Employees were exposed to serious injuries due to electrical shock, fire, or burns, in that an electrical panel located on the north wall of the second floor supply room rated at 120 Volts known to the company as LPF panel was damaged missing a hinge on the door that encloses the electrical panel.

Employees were exposed to an electrical hazard, in that the employer had not marked and identified the breakers in two electrical panels located on the north wall in the 2nd floor cleaning supply room , know to the company as N7 and N8 electrical panels.

29 CFR 1910.1200(h)(2):   The employer did not provide employees with effective information on hazardous chemicals in their work area at the time of their initial assignment and whenever a new chemical hazard the employees have not previously been trained about was introduced into their work area. Employees were not informed of the following: the requirements of this section; any operations in their work area where hazardous chemicals are present; and, the location and availability of the written hazard communication program, including the required list(s) of hazardous chemicals, and safety data sheets as required by this section.  On June 4, 2020, at a worksite located at 4142 S Noland, Independence, MO. Employees working onsite had not received training on the chemicals they were working with at the time of their initial assignment, such as, but not limited to the paints, plumbing chemicals, and cleaning chemicals.
