29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On June 2, 2020 employees were exposed to crushing hazards by the ram/die on the 513D mechanical power press. (400 ton Bliss, SE-400-108-72, serial # 30206-1). The employer did not ensure that energy control application steps to control the electrical, pneumatic and gravitational energy sources were implemented prior to the servicing task. The employer did not ensure: *Turn off or shut down the equipment. *Physically operate energy isolating devices to control energy sources and affix lockout device(s). *Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. *Verify that isolation and de-energization of the machine or equipment had been accomplished.

29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press: a) On June 2, 2020 the employer did not ensure the rear of press 513D (400 ton Bliss, SE-400-108-72, serial # 30206-1) was adequately guarded to prevent employees from accessing the point of operation. An employee suffered an amputation injury after the press operator actuated the press while the other employees hands were in the point of operation. The installed light curtains were not turned on at the time of the incident.

29 CFR 1910.217(e)(1)(i): A program of periodic and regular inspections of mechanical power press(es) was not established and followed to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment: a) On or about June 2, 2020, the employer did not ensure that periodic and regular inspections of power press 513D (400 ton Bliss, SE-400-108-72, serial # 30206-1) were conducted to include the testing the functionality of the rear light curtains. Failure to conduct regular inspections resulted in insufficient point of operation guarding to protect employees from amputation injury.
