On or about 03/24/2020, employees were exposed to the moving belt and pulley of a Cozzoli Machine Company pump assembly, in that, the guard over the pump assembly was not secured in place and affixed over the moving parts.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a. On or about March 24, 2020, at 7325 Ulmerton Road, Largo, FL, an employee suffered a work-related injury which resulted in a finger amputation. The employer was aware of the amputation on March 24, 2020. The employer notified OSHA on March 30, 2020 at 12:45 P.M. of the amputation.
