29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Carbon Monoxide listed in Table Z-1 in excess of the 8-hour Time-Weighted Average concentration of 50 parts per million: Warehouse: On or about June 9, 2020, two (2) employees were exposed to Carbon Monoxide from a propane powered Clark powered industrial forklift truck (PIT) at an 8-hour Time-Weighted Average of 221.5 and 258.5 parts per million, approximately 4.48 times the OSHA's Permissible Exposure Level (PEL) of 50 parts per million; this is the limit established to prevent carbon monoxide poisoning. Note: Acute exposure to high levels of carbon monoxide may result in symptoms of central nervous system toxicity, including (but not limited to) headache, dizziness, drowsiness, weakness, nausea, vomiting, confusion, disorientation, irritability, visual disturbances, convulsions, coma, and death. Following the acute-onset effects, delayed development of neuropsychiatric impairment may occur within 14 weeks of exposure, with symptoms, including (but not limited to) inappropriate euphoria, impaired judgment, poor concentration, memory loss, cognitive and personality changes, and psychosis.
