Employees were exposed to fall of approximately 12 feet when using the ladder to access and egress from structural steel.

29 CFR 1926.1060(b): The employer did not provide retraining as necessary so that each employee exposed to fall hazards maintained the understanding and knowledge acquired through initial training:  On or about June 9, 2020, the employer did not ensure employees were trained and understood not to use the top step of a 12 foot step ladder. Employees were exposed to fall of approximately 12 feet when using the ladder to access and egress from structural steel.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  a. On or about June 9, 2020, employees were exposed to a fall of approximately 30 feet while performing steel erection activities on a new commercial building located at 36933 Vine Street, Willoughby, OH 44094. The employees were performing detail work including bridging installation over open holes and along the perimeter of the building. The employer did not ensure employees were using fall protection.   b. On or about June 9, 2020, employees were exposed to a fall of approximately 30 feet while performing steel erection activities on a new commercial building located at 36933 Vine Street, Willoughby, OH 44094. The employee was performing detail work including bolt tightening over open holes and the employer did not ensure the employee was using fall protection.   Leetonia Steel Erectors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.760(a)(1), which was contained in OSHA inspection number 1457833, citation number 01, item number 001B and was affirmed as a final order on March 10, 2020, with respect to a workplace located at 447 Richmond Road, Richmond Heights, OH 44143.
