29 CFR 1910.147(c)(4)(ii): The employer's lockout/tagout procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance . On or about June 8, 2020, the employer's lockout/tagout procedures did not include line breaking procedures for piping that contains additive 612, a hazardous chemical. a) Procedures for ensuring contractor employee(s) are made aware of the identity of the hazardous chemical(s) in process piping. b) Procedures for ensuring that contractor employee(s) are trained on the hazards of these chemical(s). c) Procedures for ensuring that the appropriate type(s) personal protective equipment for the hazardous chemical(s) are selected and utilized by contractor employee(s).
