29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:  Worksite located at 311 Westport Road, Kansas City, Missouri- Employee was observed on the edge of the wall while hoisting up materials to the next level and was not protected from falling by the use of any means of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   Worksite located at 311 Westport Road, Kansas City, Missouri- Employee was exposed to a fall hazard in that the ladder he was using to access the upper landing surface was not extended at least 3 feet above the upper landing surface.
