29 CFR  1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  (a)	On or about June 10, 2020, an annual/periodic inspection of the energy control procedures has not been conducted for, but not limited to HVAC systems, fans, boilers, air compressors and building systems to ensure that procedures and requirements of the standard are being followed and that employees who implement energy control procedures understand their application.

29 CFR  1910.147(d)(2): An orderly shutdown of machine or equipment was not utilized to avoid any additional or increased hazards to employees as a result of equipment stoppage:   (a) Exhaust Fan #9, Building 1996 Basement: On or about May 13, 2020, the employer did not ensure the fan to the air handling intake was properly shut down, utilizing a hazardous energy control program procedure (Lockout/Tagout), exposing employee to caught-in and struck-by hazard, and amputation injury.

29 CFR  1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  On or about June 10, 2020, written machine/equipment specific energy control procedures for, but not limited to HVAC systems, fans, boilers, air compressors and other building systems,  did not document all hazardous energy sources associated with machines and equipment serviced by maintenance employees, and how to verify the energy sources have been isolated prior to performing work.  Equipment identified, but not limited to the below equipment, did not have all hazardous energy sources identified.  (a) Specific energy control procedures for all "Air Compressor" equipment used in the facilities and serviced by maintenance employees and outside contractors, did not identify pneumatic/compressed air as an energy source and how to isolate the pneumatic/compressed air.  This exposes maintenance employees to struck by injuries from compressed air.  (b) Specific energy control procedures for the four (4) boilers used in the facilities and serviced by maintenance employees and outside contractors, did not identify steam/ steam pressure as an energy source and how to isolate the steam/steam pressure prior to performing work on the boiler system.  This exposes maintenance employees to struck by hazard and burns from steam/steam pressure.  (c) Specific energy control procedures for the four (4) boilers used in the facilities and serviced by maintenance employees and outside contractors, did not identify natural gas as an energy source and how to isolate the gas prior to performing work on the boiler system.  This exposes maintenance employees to struck by hazard and burns.
