29 CFR 1926.20(f)(2): The employer did not develop hazard specific training programs to train each affected employee in the manner required by the standard relating to hazard reduction techniques and related matters. On or about June 11, 2020, during the installation of a storm sewer connection at a worksite located at approximately 751 Pearl Road, Middleburg Heights, Ohio, the employer did not ensure two employees working inside a trench, at a depth of approximately 6 feet, were protected from cave-ins by an adequately designed protection system. The soil would have been classified as; Cohesive Soil, Type B due to it was previously disturbed and subject to vibration for road traffic.
