The employer required employees performing work, such as, but not limited to, Janitorial, Maintenance, Administration and Patient care, to wear respirators, including, but not limited to, KN95 respirators, throughout their shifts while exposed to suspected or confirmed positive COVID-19 patients.

29 CFR 1910.134(k)(1): The employer did not provide effective training that covered the required elements in 1910.134(k)(1)(i) - (vii): (a) Throughout the Willowood Care Center of Brunswick, Inc. facility located at 1186 Hadcock Rd., Brunswick, Ohio 44212: On or about June 9, 2020, the employer failed to train employees who were required to wear a KN95 respirator while performing work, such as but not limited to, Janitorial, Maintenance, Administration and Patient Care. The employer failed to ensure each employee could demonstrate knowledge of at least the following: i) Why the respirator is necessary and how improper fit, usage, or maintenance can compromise the protective effect of the respirator. 1910.134(k)(1)(i) ii) What the limitations and capabilities of the respirator are. 1910.134(k)(1)(ii) iii) How to use the respirator effectively in emergency situations, including situations in which the respirator malfunctions. 1910.134(k)(1)(iii) iv) How to inspect, put on and remove, use, and check the seals of the respirator. 1910.134(k)(1)(iv) v) What the procedures are for maintenance and storage of the respirator. 1910.134(k)(1)(v) vi) How to recognize medical signs and symptoms that may limit or prevent the effective use of respirators. 1910.134(k)(1)(vi) vii) The general requirements of 1910.134. 1910.134(k)(1)(vii)
