29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about 06/10/2020 at the Rent-A-Center Roof Repair Project, an employee was working from the roof deck and was exposed to an 18 foot fall hazard while installing an EPDM roofing membrane. No fall protection system was in use.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about 06/10/2020 at the Rent-A-Center Roof Repair Project, an employee was using a screw gun to fasten the insulation to the deck and the screw gun was energized by an extension cord that had strain relief damage.
