29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5). On and before June 6, 2020, employees exposed to struck-by and crushing hazards while employees operating a Teledyne Princeton Forklift, had not completed power industrial truck training provided by the employer per the requirements of the standard.

29 CFR 1910.178(n)(6): The driver shall be required to look in the direction of, and keep a clear view of the path of travel. On June 6, 2020, located on a residential construction site, an employee operating a Teledyne Princeton Forklift transporting sod to a front yard failed to keep a clear view path in that a pallet of sod was obstructing the view exposing employees to struck-by and or crushed-by hazards.
