29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) Worksite:  On or about 6/11/2020, an employee working in a 10 foot deep trench was not wearing a protective helmet, exposing the employee to falling objects from above.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins or indications of failure of protective systems:  (a) Worksite:  On or about 6/11/2020, the employer did not have a competent person perform daily inspections of an approximate 10 foot deep excavated area to ensure that protective systems were in place to prevent possible cave-ins.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):  (a) Worksite:  On or about 6/11/2020, an employee was found working at the bottom of a trench that measured approximately 6' 2" wide by 12' 8" long by 10' deep, without shoring or a trench box, to prevent cave-ins or collapse of the trench.
