29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) On or about 6/11/20, at the site of Buffalo, NY. Employees did not wear personal protective equipment such as: flame resistant clothing, during the process of adding water into a reactor kettle of heated oil and melted stearic acid. Once water was added, a strong reaction produced the heated oil and melted stearic acid solution to come out of reactor kettle burning employees on their back, arms, and legs. ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): a) On or about 6/11/20, at the site of Buffalo, NY. The employer did not assess the hazards such as, but not limited to: chemical/hot solutions splashes that may be present in the process of making grease in a reactor kettle, and did not provide the necessary PPE to protect exposed employees. Employees working on the production of grease did not wear any PPE and were exposed to severe burns. ABATEMENT CERTIFICATION REQUIRED
