29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about June 15, 2020, during the exterior demolition of a commercial building, located at 34881 Euclid Avenue, Willoughby, Ohio, the employer did not ensure an employee working from a JLG Model 600AJ aerial lift was wearing and utilizing a body belt (harness) and lanyard attached to the basket of the aerial lift.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about June 15, 2020, during the removal of roofing material for the demolition and remolding of a commercial building located at 34881 Euclid Avenue, Willoughby, Ohio, the employer did not ensure two employees working from a low-sloped roof were protected from a fall of approximately 15 feet by the use of a guardrail system, safety net system, personal fall arrest system and/or combination warning line system and safety monitoring system.
