The employees were working on all areas of the roof and the employer did not ensure they were using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS) On or about June 16, 2020, employees were exposed to a fall of approximately 9 feet while performing roofing activities on a residential home located at 502 Smith Road, Medina, Ohio 44256. The employees were using an extension ladder to access the roof that did not extend three feet above the landing area.
