29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    Employees engaged in residential framing activities using a Hitachi pneumatic nail gun model number NV 45AB2, were exposed to struck-by hazards in that eye protection was not provided.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    Employees were exposed to fall hazards.  Employees engaged in residential roofing activities, walking and working on a 6:12 pitch roof, were exposed to fall hazards of approximately 16 feet from the eave of the building to the ground below in that fall protection was not utilized.

29 CFR 1926.1053(b)(1): 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    Employees were exposed to fall hazards.  Employees engaged in residential roofing activities, using a Werner 24 foot metal extension ladder, model #D1224-2, were exposed to fall hazards of approximately 11 feet from the eave of the building to the ground below. Employees were exposed in that the ladder was not secured to the building and did not extend at least 3 feet above the eave.
