29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial trucks were performed by the employer without the manufacturer's prior written approval: a) On or about 06/16/2020, at the warehouse at the Palmetto Port, forklifts were used with attachments and slings to pick up totes containing salt weighing approximately 1.1 tons each. The employer had not obtained written approval from the manufacturer regarding the safe usage of these forklift attachments.
