29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Exterior Front (Lower Roof): An employee was not wearing a protective helmet, while engaged in roofing work, was exposed to a struck by hazard to the head from employees performing roofing work above. The employer did not ensure that employees were provided with protective helmets, thus exposing employees to a head laceration, on or about 6/17/20.  b) Exterior Front (Ground Level): Employees working on the ground level and below roofing work operations, were not wearing protective helmets. The employer did not ensure that employees working on the ground level, were provided with protective helmets, thus exposing employees to a head laceration, on or about 6/17/20.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  a) Exterior Front: Employees engaged in roofing work utilized hand-held pneumatic nail guns, without eye and face protection. The employer did not ensure that employees using hand held pneumatic nail guns were protected with eye and face protection, thus exposing employees to an eye injury, on or about 6/17/20.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Exterior Front (Upper Roof): Employees engaged in roofing work on the upper roof level were exposed to a fall hazard of approximately 28 feet above the lower surface, without the use of a personal fall arrest system. The employer did not ensure that employees performing roofing work were provided with a personal fall arrest system, or other means of fall protection, thus exposing employees to a fall hazard, on or about 6/17/20.  b) Exterior Front (Lower Roof): An employee engaged in roofing work on the lower roof level was exposed to a fall hazard of approximately 20 feet above the lower surface, without the use of a personal fall arrest system. The employer did not ensure that employees performing roofing work were provided with a personal fall arrest system, or other means of fall protection, thus exposing employees to a fall hazard, on or about 6/17/20.
