29 CFR 1926.451(c)(1)(ii): Guys, ties, and/or braces were not installed  according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less therafter for scaffolds greater than 3 feet (0.91m) wide.  (a) Rear of building - On or about June 16, 2020 The employer did not ensure the scaffolding being used by employees to apply stucco to building was secured to the building, exposing employees to fall hazards.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  (a) Rear of building - On or about June 16, 2020 the employer did not ensure the scaffolding was fully planked prior to having employees work on it, exposing employees to fall hazards.  Hermellio Ortis was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1) which was contained in OSHA inspection number 1340505, citation number 1, item number 1 and was affirmed as a final order on November 23, 2018, with respect to a workplace located at 776 W. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  (a) Rear of building - On or about June 16, 2020 the employer did not provide a means for the employees to use while gaining access to the scaffolding, exposing employees to fall hazards.
