29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about June 17, 2020, at a worksite located at 2644 SOM Center Road, Willoughby Hills, OH 44094 during residential roofing activities employees were exposed to fall hazards of approximately 10.2 feet. The employer failed to ensure fall protection systems/devices were used by all employees working on the roof.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:   On or about June 17, 2020 during residential roofing activities, the employer did not have a safety and health program implemented at the worksite. The employer did not ensure employees were provided safety and health training or that there was a system in place to recognize and correct unsafe working conditions.
