29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  A and H Roofing Services, LLC located at 2315 Canterbury Rd Upper Arlington, OH 43221: One and preceding June 17, 2020, employees were performing roof work on a two story residential home at heights up to 25 feet without fall protection, PPE and improperly configured ladders. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted by employees.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  A and H Roofing Services, LLC located at 2315 Canterbury Rd Upper Arlington, OH 43221: One and preceding June 17, 2020, the employer did not instruct employees in the recognition and avoidance of unsafe conditions and the procedures to be followed in order to minimize these hazards. Roofers were installing shingles and other roofing materials on the second floor at heights up to 25 feet. The employees were not utilizing proper fall protection, set a ladder up improperly and were not instructed on the use of PPE which exposed them to falls up to 25 feet and struck by hazards to the eyes.

Roofers were installing shingles and other roofing materials at heights up to 25 feet without fall protection. The following condition exposed them to falls up to 25 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  A and H Roofing Services, LLC located at 2315 Canterbury Rd Upper Arlington, OH 43221: One and preceding June 17, 2020,  the employer did not ensure that the ladder used for access to the roof extended at least three feet beyond the surface they were accessing. Roofers were using an extension ladder to access the roof of a two story residential home without the ladder properly extended or secured to the house. The following condition exposed them to falls up to 12 feet.
